SCOH Social Worker

Summary:

The SCOH Social Worker is responsible for providing services to and on behalf of children and families referred for SCOH Services through the Family Services Department. This position requires working knowledge of community resources, physical and behavioral health services and educational system. The social worker should institute the principles of ASFA and TANF (i.e. safety and employment). The social worker is responsible to coordinate and advocate for services on behalf of the family.

• Responsibilities include documentation, planning, monitoring, communication, direct care and networking.

• Working knowledge of the Child Welfare System and ability to navigate other systems as well as knowledge of the use of computers and pertinent software programs is required.

Requirements:

Education: Minimum requirement is a Bachelors degree.

Experience: Experience in child welfare. Working knowledge of clinical issues relating to children, adolescents and/or families is required.

Special Skills: This position requires a valid driver’s license and travel through the five county areas and beyond, ability to lift 20 lbs., ability to perform SPMCI techniques and to walk up and down stairs.

SCOH Social Worker Level II

Summary:

This is a direct practice and/or supervisory position. Under the supervision of the SCOH Supervisor, the SCOH Level II Social Worker assists in the provision of informal supervision and direction to social work staff to assure all SCOH services are delivered according to the highest quality clinical standards and according to regulatory and funding source guidelines. Helps provide leadership, planning, clinical support and administrative functions necessary to implement principles of best practice. Monitors program implementation by social workers. Based on need and availability the SCOH Social Worker II functions as a SCOH Social Worker with a limited caseload.

• Additional responsibilities for the position include: training of new employees; participation in weekly admission and supervisors meetings; assistance with completion of Level 2 Reviews quarterly for Clinical Standards and assistance with new initiatives or program improvement projects.

Qualifications:

Education: Bachelor’s degree in Human Services Field.

Experience: 6 months experience (minimum) working as a SCOH Social Worker with Northern Home; 1 year or more preferred.

Excellent performance as indicated in performance evaluations.
