Foster/Kinship Care Case Manager

Summary:

Under the direction of the Foster/Kinship Care Supervisor, the foster/kinship care social worker is responsible for providing services to and on behalf of the consumers, natural families and foster families referred to the foster/kinship care program.

The Case Manager is responsible for taking every necessary step to ensure that the physical, emotional, social, intellectual and safety needs of the consumer are being met and maintained in accordance with policies and procedures of current state and federal standards, guidelines and regulations.

The position establishes effective relationships with the Department of Human Services (DHS), Community Behavioral Health (CBH), Family Court systems, Philadelphia School District, Northern Home’s admissions department as well as other internal and/or external parties to ensure quality service delivery.

In addition to these responsibilities, the incumbent assures that the department is appropriately represented at meetings and acts as liaison to appropriate service systems when necessary.

The foster/kinship care case manager has a thorough understanding of the regulations and contracts guiding the work of the foster/kinship care program.

Requirements:

Skills & Qualifications:

  • Ability to communicate and interact with all staff.
  • Ability to define problems, collect data, establish facts and draw valid conclusions.
  • Ability to effectively present information and respond to questions from groups of managers, consumers, vendors, and the public.
  • Ability to give clear and concise oral and written instructions.
  • Working knowledge of the Child Welfare System.
  • Knowledge of and ability to navigate other systems.
  • Ability to read and understand regulatory policies.
  • Working knowledge of the use of computers and pertinent software programs.
  • Requires some flexible work schedule, evening and weekend hours.
  • Required to carry emergency pager on a rotating basis.

Education: Bachelors Degree required; Master’s preferred.

Experience:

  • Minimum of one (1) year experience in child welfare, or social services field, acting in a case management capacity, and working knowledge of clinical issues relating to children, adolescent, and/or families.
  • Experience working with the Philadelphia Department of Human Services and under Performance Based Contracting desirable
