Community Based Prevention Case Manager/Liaison

Summary:

This mental health clinician provides behavioral prevention, intervention and consultation services to members of the community in the Strawberry Mansion and Central North section of Philadelphia. The Community Based Prevention Case Manager/Liaison facilitates the identification of high-risk youths and families and provides interventions and linkages to community based services.

Under the supervision of the Manager of the Community Based Services Department, the Community Based Prevention Case Manager/Liaison assures that all services are delivered according to the highest quality clinical standards and according to regulatory and funding source guidelines. The Liaison is also responsible to maintain accurate data base information on service delivery and client information.

Qualifications:

Education: Minimum-Bachelors Degree

Experience: Minimum of two-years working with children, adolescents and families in a CASSP/Behavioral Health system, including one-year experience working within a school setting.

Minimum of 2-year experience with Behavioral Intervention

Special Skills: Prepare and monitor paperwork. Knowledge of Behavioral Health System.
