Care Coordinator
- Under the supervision of a Clinical Specialist the Care
Coordinator assists consumers in gaining access to necessary
medical, educational, rehabilitative, and social services.
The Care Coordinator is to assure that the consumers receive
services identified on the psychiatric and psychological
evaluation throughout the duration. The Care Coordinator
is expected to work closely with other disciplines to create
the best treatment program for the designated cases and
must customize service to meet the needs of the individual.
Additionally the Care Coordinator assures that the consumer's
needs are coordinated, among other program, agency departments,
outside agencies and funding source.
- Ability to communicate and interact with all staff; to
define problems, collect data, establish facts and draw
valid conclusions; to effectively present information and
respond to questions from varied groups, including the media;
to accurately calculate numbers such as in addition, subtraction
and percentages and to give clear, concise, oral and written
instructions.
- Working knowledge of the Behavioral Health system and
ability to navigate other social service systems as well
as knowledge of the use of computers and pertinent software
programs
Education: Minimum requirement is a Bachelor's
Degree.
Experience: Two years case management of
counseling experience with children and families.
Special Skills: Familiar with multiple child-serving
systems (e.g. education, juvenile justice, child welfare,
mental health, drug and alcohol, health care, and vocational
rehabilitation).
Other Requirements: Job demands the ability
to work independently and cooperatively as part of a team.
Job requires the ability to manage work-related stress. Must
demonstrate and maintain good writing skills.
